Cataract Surgery

In cataract surgery, the lens inside your eye that has become cloudy is removed and replaced with an artificial lens (called an intraocular lens, or IOL) to restore clear vision. This is generaly made up of acrylic these days. 

We discuss and evaluate the functional difficulty with glare test, measure your eyeball size to get you ready for cataract surgery. Based on these information an appropriate prescription  clear intraocular implant is chosen to replace your natural cloudy lens ( cataract) . 

Dr Alizai will discuss with you various distance options that you may prefer in the activities of daily living i.e whether your goal is distance, near or intermediate distance without glasses , in case you elect the monofocal IOL, which most insurances allow .

There are other fancier upgrade options for multiple distances including Multifocal or Extended depth of focus intraocular lenses with or without astigmatism correction, if preferred - the insurances generally don’t cover the upgraded IOL.  Ask Dr Alizai -Ansari about all these options or upgrades whether those are reasonable choices ......... to choose the one best for your visual needs including the pros and cons.

Pterygium Surgery

Some elderly patients lose elasticity of the eyelids creaing loose hanging skin of the upper eyelid that interferes with their vision and upper peripheral visual field. 

Others have excessive tearing that requires them to wipe their eyes more than several times a day as tears dont drain properly. 

Upper eylid lid  tightening  or functional blepharoplasty and ptosis surgery are eyelid tightening procedures maybe medically necessary services and have a criteria outlined by medicare and other insurance guidelines. Ask us if you are suffering from any of  these eand need evaluation and treatment . We may perform photos or ptosis visual fields in the office to see if you qualify and Dr Alizai will suggest which would be the best option for you.

Some patients require opening up of the tightened tear draining channels that connect the lower eyelid to the nose. It is the plumming system for your tears. It is an in office and painless procedure. Ask Dr Alizai- Ansari about this procedure.

 

 

Pterygium surgery involves removal of the abnormal tissue from the sclera and cornea of the eye. Today's techniques offer a significantly higher success rate than conventional surgery. In traditional “bare sclera” pterygium removal, the underlying white of the eye is left exposed. In other cases amniotic membrane graft is placed on the conjunctival defect after pterygium excision.

  

 

 

Glaucoma Management and Screening

Primary open-angle glaucoma (POAG) is a leading cause of blindness and vision-related disability. This review examines the effectiveness of screening and treatment of asymptomatic individuals with early POAG.

You may be asked to take the visual field test to look for peripheral visual field blind spots or OCT of the optic nerve which is a scan of the optic nerve which is the main structure of eye affected by glaucoma and other optic nerve diseases . Glaucoma is generally treated with 1-3 eye drops in most patients . Some patient require laser (SLT) or glaucoma surgery , in case it is not well controlled . Dr Alizai also specializes and offers MIGS surgical procedures along with cataract surgery or by itself, if indicated for mild, moderate or severe glaucoma .

 

 

 

Dry Eye Evaluation and Management

Dry eye syndrome is a common ocular condition often under-diagnosed.  Symptoms can include foreign body sensation, burning, redness, intermittent blurry vision and contact lens intolerance.  Signs of dryness can be found on routine ocular exam by your ophthalmologist.  If you believe you may be suffering from dry eye, our ophthalmologists can evaluate and treat you. One can consider taking fish oil supplements and improve hydration. Over the counter artificial tears and gel tears at bedtime are other options. Dr Alizai could suggest other dry eye prescription medications, if needed. We may measure the quantity of tears produced during your exam in the office.

 

 

 

Macular Degeneration Screening and Management

Macular degeneration is a deterioration of the central retina called the macula or the fine focusing region of the central part of the inside of the eye. The macula is responsible for your central vision and the ability to see color and fine detail.

There are two forms of macular degeneration, dry and wet. With dry macular degeneration a breakdown in the retinal pigment epithelium occurs, deposits called drusen develop in the retina and the tissue becomes thin and stops functioning properly.

As the disease slowly progresses, central vision diminishes. This degeneration may be due to the natural aging process, increased UV exposure, smoking, poor diet and/or genetics (family history), racial predisposition. Treatment consists of nutritional management, cessation of smoking, and UV protection. With wet macular degeneration fluid leaks from delicate abnormal blood vessels which have formed at the macula. This form is more serious and vision can decrease more rapidly. In recent years, new treatments consist of anti-vegF injections in the eye which reduce the fluid and swelling  in the retina .

 

Diabetic Retinopathy Screening and Management

The American Academy of Ophthalmology recognizes that screening for diabetic retinopathy using validated digital imaging can be a sensitive and effective detection method. Such technology has not been demonstrated to be as effective, however, at detecting and quantifying the spectrum of other ophthalmic pathology that can accompany diabetic retinopathy, including cataracts and glaucoma , which are more prevalent in patients with diabetes mellitus and can cause blurry vision , glare or halos.  

Diabetic patients may have an earlier and faster deterioration in their cataracts, especially, if poorly controlled .

Imaging technology also does not mitigate the need for periodic comprehensive ophthalmic examinations by an eye care professional.  Most insurances encourage and pay for at least annual diabetic eye exams for your eye health maintenance.

If your eyes itch and are red, tearing or burning, you may have eye allergies (allergic conjunctivitis), a condition that affects millions of Americans. Many people will treat their nasal allergy symptoms but ignore their itchy, red, watery eyes. Chronic tearing could also be a result of blockade of your tear drainage system which may require opening and could be a plumbing issue. Various medications or diseases may affect tear formation and drainage.

We evaluate serious brain related causes of very severe headaches that your neurologist may not have been able to help you with. First a brain tumor other brain masses have to be ruled out by  brain imaging studies. 

We evaluate you for swelling on your optic nerve by doing a dilated eye exam which maybe an early sign of increased intracranial or brain fluid pressure. Patients may require brain imaging including MRImaging / MRVenogram  or spinal tap to check brain fluid pressure and formula.
